| dc.description.abstract | This study was designed to develop a well-organized resource guide in Microsoft Teams for North Shore Physicians Group community health workers (CHWs). The resource guide was designed to aid community health workers with day-to-day tasks by reducing time researching resources in databases. The goal was to promote more efficient patient-centered care. Seven CHWs and 30 patients were recruited to participate during the community van peak hours. Results indicated that resources for transportation and housing assistance were the most common requests. The outcome of the project was a successful resource for community health workers which met their need for a comprehensive and organized tool that allows them to use their time more efficiently to better serve their patients. | |
