    Abstract
    This project brings out the different ways heart failure is managed through a multidisciplinary care team approach. The different authors used evidence-based studies, randomized control trials, focused group studies, qualitative studies, descriptive literature review and synthesis, descriptive, correlative review and cohort studies to describe the different ways heart failure patients manage their disease to prevent rehospitalization, which is very common within the first thirty days after discharge from the hospital. All articles are current, and focused on heart failure and are derived from CINAHL database, with focus on nursing topics and allied health science. The literature review is generated from primary articles which are the articles used in this study. The PICOT format helps to bring out the population group involved in the study, the summary of possible interventions in heart failure management, comparison intervention, outcome, and the time frame the study was conducted. The PICOT approach helps to answer the clinical question “How effective is self-care management of heart failure?” All articles used in this study are summarized on critical appraisal tables 1 and 2, with vivid and brief description of the literature review of heart failure self-care management.
